2. Transcript of grades. An official transcript of grades for all high school work completed. A minimum cumulative grade point average of 2.50 is required.

3. Two letters of references. Comments on your character and potential, as well as school, community service, leadership, artistic, personal, athletic, and/or Hawaiian cultural activities should be included. Ask the people writing the letters to include their name, address, phone number and professional title, and to give you their letter in a sealed envelope for you to submit with your application.

4. A recent original photograph of you individually. Please write your name on the back of it.

5. If you are a dependent, your parent(s) most recent federal tax return. If you are not being claimed as a dependent for tax purposes, your most recent federal tax return. Please blacken out all social security numbers.

To qualify an applicant must:

1. Demonstrate financial need. Judging is based on the current poverty level guidelines.

2. Be a resident of Hawaii.

3. Be a graduating high school senior. This can also include homeschoolers if they are graduating through the auspices of a Hawaii Department of Education high school, Hawaii public charter school, or member of the Hawaii Association of Independent Schools.

4. Plan to attend a college, university, technical institute, professional training program, union apprenticeship, or other valuable academic and job training opportunity.
