Revised 04/09

Student Leadership Recommendation Form

Name of applicant: ______________________________________________________________________________________

How do you know the applicant? For how long?

Does this candidate show good leadership skills?

Does this candidate communicate well?

Does this candidate show initiative and responsibility?

This position requires a commitment to the Student Leadership role. This person must act as a role model to other students. It is not a “job” that ends at a certain time. As I tell them, “you are always a Student Leader, on campus or off.” Knowing this, how would you recommend this applicant:

◊ Strongly recommend. This candidate would do a wonderful job.

◊ Recommend with slight reservations. This candidate would do well with training.

◊ Do not recommend. This candidate is not ready for this kind of responsibility.
